New Trends in Craniovertebral Junction Surgery: Experimental and Clinical Updates for a New State of Art

cover
Author(s): Massimiliano Visocchi
Series: Acta Neurochirurgica Supplement 125
Publisher: Springer International Publishing, Year: 2019
ISBN: 978-3-319-62514-0,978-3-319-62515-7
Description:

This issue of Acta Neurochirururgica presents the latest surgical and experimental approaches to the craniovertebral junction (CVJ). It discusses anterior midline (transoral transnasal), posterior (CVJ craniectomy laminectomy, laminotomy, instrumentation and fusion), posterolateral (far lateral) and anterolateral (extreme lateral) approaches using state-of-the-art supporting tools. It especially highlights open surgery, microsurgical techniques, neuronavigation, the O-arm system, intraoperative MR, neuromonitoring and endoscopy.
Endoscopy represents a useful complement to the standard microsurgical approach to the anterior CVJ: it can be used transnasally, transorally and transcervically; and it provides information for better decompression without the need for soft palate splitting, hard palate resection, or extended maxillotomy. While neuronavigation allows improved orientation in the surgical field, intraoperative fluoroscopy helps to recognize residual compression. Under normal anatomic conditions, there are virtually no surgical limitations to endoscopically assisted CVJ and this issue provides valuable information for the new generation of surgeons involved in this complex and challenging field of neurosurgery.

The atlas illustrates several different approaches to adrenalectomy, which include more commonly used anterior lateral adrenalectomy and more advanced newer techniques of posterior retroperitoneoscopic and robotic adrenalectomies. This atlas emphasizes different approaches and ways to perform the same procedure. Specifically how the same procedure could be successfully completed by different surgeons with specific details of each technique. Each approach is presented by three different expert-surgeons illustrating three different techniques of procedure, including single-site approach. Different surgeons have different “tricks” on how to perform this type of procedure successfully. Each chapter presents procedures that are performed and illustrated by a different single surgeon. All authors are members of American Association of Endocrine Surgeons (AAES) and are experts in this specific technique. This atlas includes 18 chapters and illustrates traditional open approach, anterior lateral right and left adrenalectomy approaches, and more advanced state-of the-art posterior retroperitoneoscopic right and left adrenalectomies (including single port procedure) and robotic adrenalectomies. The text includes extensive illustrations and links to video of all procedures making this an interactive text.
Atlas of Adrenal Surgery represents the first single source to provide information on technique and outcomes for a diversity of endocrine surgeons, minimally invasive laparoscopic surgeons, surgical oncologists, urologists and general surgeons.

Publisher: Springer International Publishing, Year: 2019
ISBN: 978-3-030-01164-2,978-3-030-01165-9
Description:

Colorectal Surgery has continued to experience tremendous growth in both the community and academic settings over the past few years. The recent increase in demand for colorectal specialists has been fueled by an overwhelming number of applications to fellowship training programs, resulting in some of the most coveted and competitive positions. Furthermore, the accumulation of experience, knowledge, and wisdom from pioneers in the field, combined with major recent technological advances, has transformed the clinical management of diseases of the colon and rectum. Colorectal Surgeons have embraced advances ranging from minimally invasive approaches for complex problems to novel training methods for future generations. Additionally, we have spearheaded innovations in the management of colorectal cancer, pelvic floor disorders, diverticulitis, inflammatory bowel disease, and anorectal conditions. Despite these improvements, there remains a seemingly never-ending mixture of complex patient disease processes and complications resulting from the care of these patients. Even in cases where the technical challenges were managed successfully, complications or poor function may result in dramatic life-long consequences, reduced quality of life, as well as having economic implications.
The American Society of Colon and Rectal Surgeons (ASCRS) is the premiere professional organization of Colon and Rectal Surgeons. Three editions of the ASCRS Textbook of Colon and Rectal Surgery have been published and have proved to be extremely valuable for their wealth of general information and knowledge, providing not only background information, but also specifics regarding the more complex situations that surgeons who treat patients with colorectal disease experience on a regular basis.
An ASCRS manual was produced in in 2009 and 2014, each accompanying their original textbooks. This has been formed by abstracting the textbook into a bullet format; all figures and most tables were retained. The 3rd edition of the Textbook (published by Springer) included completely new chapters and authors. This 3rd edition of the Manual is indicated to conform to the new edition of the Textbook and incorporate newer information in the field of colon and rectal surgery. This Manual will serve as a very useful resource for physicians and researchers dealing with diseases of the colon and rectum. It will provide a concise yet comprehensive summary of the current status of the field that will help guide education, patient management and stimulate investigative efforts. All chapters were written and abstracted by experts in their fields and will include the most up to date scientific and clinical information.

Publisher: Picador, Year: 2019
ISBN: 1509837027,9781509837021
Description:
For more than twenty-five years, David Nott has taken unpaid leave from his job as a general and vascular surgeon with the NHS to volunteer in some of the world’s most dangerous war zones. From Sarajevo under siege in 1993, to clandestine hospitals in rebel-held eastern Aleppo, he has carried out life-saving operations and field surgery in the most challenging conditions, and with none of the resources of a major London teaching hospital.

The conflicts he has worked in form a chronology of twenty-first-century combat: Afghanistan, Sierra Leone, Liberia, Darfur, Congo, Iraq, Yemen, Libya, Gaza and Syria. But he has also volunteered in areas blighted by natural disasters, such as the earthquakes in Haiti and Nepal.

Driven both by compassion and passion, the desire to help others and the thrill of extreme personal danger, he is now widely acknowledged to be the most experienced trauma surgeon in the world. But as time went on, David Nott began to realize that flying into a catastrophe – whether war or natural disaster – was not enough. Doctors on the ground needed to learn how to treat the appalling injuries that war inflicts upon its victims. Since 2015, the foundation he set up with his wife, Elly, has disseminated the knowledge he has gained, training other doctors in the art of saving lives threatened by bombs and bullets.

Female Genital Cosmetic Surgery: Deviance, Desire and the Pursuit of Perfection

Description:

Examining the fascinating history of female genital cosmetic surgery, Camille Nurka traces the origins of contemporary ideas of genital normality. Over the past twenty years, Western women have become increasingly worried about the aesthetic appearance of their labia minora and are turning to cosmetic surgery to achieve the ideal vulva: a clean slit with no visible protrusion of the inner lips. Long labia minora are described by medical experts as ‘hypertrophied,’ a term that implies deformity and the atypical. But how far back does the diagnosis of labial hypertrophy go, and where did it originate? Female Genital Cosmetic Surgery tells the story of the female genitalia fromthe alien world of ancient Greek gynaecology to the colonial period of exploration and exploitation up to the present day. Bringing together historical, medical, and theoretical documentation and commentary, Nurka uncovers a long tradition of pathologizing female anatomy, a history sure to be of interest to any reader who wishes to know more about how medicine shapes our commonly held ideals.

Publisher: Springer International Publishing, Year: 2019
ISBN: 978-3-319-90751-2,978-3-319-90752-9
Description:
As minimally invasive surgical techniques evolve, the outlook for patients continues to brighten. This is also true for pregnant women experiencing conditions requiring surgical intervention during gestation. A physician’s ability to identify potential risk factors in pregnant patients that present pre-op directly corresponds with their success in monitoring patients in post-op for adverse obstetric outcomes from non-obstetric surgeries. The utilization of laparoscopic techniques during surgery for pregnant patients has decreased the risk of fetal complications and increased the positive outlook for the duration of the pregnancy.
This text is designed to present a minimally invasive approach to surgery that is non-obstetrics related and to educate readers on the potential risk factors and negative outcomes on pregnant patients following non-obstetric surgery. The various sections of this book will address issues faced by surgeons who undertake the task of operating on the pregnant patient. When performing non-obstetric surgery on a pregnant patient the ability to identify possible risk factors for adverse obstetric outcomes is important. Risk factors can be maternal-, surgical-, obstetrical-, or disease-associated. Common adverse obstetric outcomes following non-obstetric surgery include preterm delivery, preterm labor without preterm delivery, and miscarriage. Fetal well-being is an additional risk-benefit factor to be considered when weighing options before conducting non-obstetric surgeries on pregnant patients. This book will provide physicians with the knowledge and tools to identify common risk factors and successfully apply evidence-based risk reduction.
To date, no comprehensive resource is available for non-obstetric surgery during pregnancy. Types of pregnancies, potential complications, proper use of anesthesia, types of risk factors (maternal-, surgical-, obstetrical-, and disease-associated), etc. will be thoroughly explained and depicted within these pages. All chapters will be written by subject matter experts in their fields. All information communicated will be comprised of the most currently available knowledge

Publisher: Springer International Publishing, Year: 2019
ISBN: 978-3-030-03019-3,978-3-030-03020-9
Description:

The introduction of a new technology in a consolidated field has the potential to disrupt usual practices and create a fertile ground for errors. An example is robotic surgery that is now used in most surgical specialties, pushed by technology developers and enthusiastic surgeons. To analyze the potential impact of robotic surgery on patient safety, a consortium of major European Universities started the project SAFROS whose findings are summarized and further elaborated in the three parts of this book. Part one describes safety in complex systems such as surgery, how this may disrupt the traditional surgical workflow, how safety can be monitored, and the research questions that must be posed. Part two of the book describes the main findings of this research, by identifying the risks of robotic surgery and by describing where its ancillary technologies may fail. This part addresses features and evaluation of anatomic imaging and modeling, actions in the operating room, robot monitoring and control, operator interface, and surgical training. Part three of the book draws the conclusions and offers suggestions on how to limit the risks of medical errors. One possible approach is to use automation to monitor and execute parts of an intervention, thus suggesting that robotics and artificial intelligence will be major elements of the operating room of the future.
